STUDENT INFORMATION AND CONSENT FORM

Ensuring that we have accurate up to date information about our students is one of our top priorities. Not only is having accurate and update to date contact and medical information important to us but we also want to ensure we have the correct permissions in regards to the use of media such as photos on our online platforms. (school website, Class Dojo and etc)

As a new starter at Alway Primary School it is a requirement to provide us with these details and we recommend that parents fill in the form at least once every year to ensure that the information is still up to date. Throughout your child's time here at the school you can update the information as much as you need to.
